Data Dictionary Demo - Stroke

Welcome to the Stroke Data Dictionary Demo

This demonstration provides standardized forms for collecting stroke-related data according to the ICHOM Stroke Data Dictionary.

About the Stroke Data Dictionary Demo

This demonstration showcases the comprehensive data collected with the ICHOM Stroke Data Set. The forms in this system are designed to demonstrate the types of data collected about stroke patients, their treatments, and outcomes. The data dictionary has been organized into separate forms based on the reporting source to make data collection more efficient and targeted for different stakeholders in the stroke care process.
