About the Stroke Data Dictionary Demo
This demonstration showcases the comprehensive data collected with the ICHOM Stroke Data Set. The forms in this system are designed to demonstrate the types of data collected about stroke patients, their treatments, and outcomes. The data dictionary has been organized into separate forms based on the reporting source to make data collection more efficient and targeted for different stakeholders in the stroke care process.
Instructions for Use
Please select the appropriate form based on your role in the stroke care process and the type of data you are reporting. Each form contains fields specific to that reporting source category. Required fields are marked with an asterisk (*).
- Patient-reported forms: To be completed by patients or their caregivers
- Clinical forms: To be completed by healthcare providers
- Administrative forms: To be completed by administrative staff
- Combined forms: For data that may come from multiple sources
After completing a form, click the "Submit" button to save the data. You can also print the form for your records or reset the form to clear all entered data.
